Does the car need to be running for it to work?

The controllers will work whenever the car sending power to the valves, that can include with the ignition on without starting the engine.

The car does have to be powered on for the controllers to work, but does not always mean the engine has to be running. The car powers the valves before the engine is started and sometimes just by unlocking the car.

This depends on the make and model.
